Simple BIP Team Collaboration Form

Use this form with one or more members of a student's team to help develop a Simple BIP in Behavior Advantage.  Simple BIP's should include informal team discussions exploring the environmental and functional variables affecting behaviors (e.g., triggers, function, skill deficits, etc.).  The Simple BIP Team Collaboration Form can be used to record and organize information from these team discussions.  Follow the instructions after each section to develop a Simple BIP in Behavior Advantage that matches the individual's specific and functional needs.
